What is Health & Wellness Self-Management?
Health & Wellness Self-Management focuses on teaching individuals practical skills to better understand their health, manage daily wellness routines, and navigate healthcare systems with confidence. Services include education, coaching, and support — not medical treatment.
Who provides the services?
Services are provided by trained staff under the leadership of a Registered Nurse with extensive experience in healthcare, community-based support, and health education.
Is this medical care or nursing treatment?
No. WiseCare Health Support provides health education and self-management coaching only. We do not diagnose conditions, prescribe medications, or provide medical treatment. Our services do not replace care from a doctor or other licensed healthcare provider.
Do I need health insurance to receive services?
No. These services are private-pay and do not require insurance. This allows us to focus on education, coaching, and flexibility based on your needs.
Do you accept Medi-Cal, Medicare, or private insurance?
At this time, WiseCare Health Support does not bill insurance. We offer affordable, transparent pricing so clients can choose the level of support that works best for them.

How often are sessions held?
Session frequency depends on the service package you choose. Options range from bi-weekly to weekly sessions, with additional check-ins available as needed.
Can you help me understand my medical condition?
Yes. We provide plain-language education to help you understand your health conditions, common symptoms, and warning signs — so you can communicate more effectively with your healthcare provider.
Can you come with me to my medical appointments?
We help you prepare for appointments and understand follow-up instructions. Physical accompaniment may be available depending on location and scheduling; please contact us to discuss your needs.
Can you help me manage my medications?
We provide medication education, such as understanding the purpose of medications and creating reminders. We do not adjust doses or make medical decisions.
Do you help with nutrition and grocery shopping?
Yes. We offer budget-friendly nutrition education, meal planning, grocery shopping skills, and CalFresh (food stamp) education. We do not prescribe special diets.
Can you teach me how to check my blood pressure or blood sugar?
Yes. We can teach you how to safely check and track vitals such as blood pressure, heart rate, weight, and blood sugar (for individuals already prescribed monitoring devices). We do not interpret lab results or change treatment plans.
Do you help with emergency planning?
Yes. We teach how to recognize emergencies, when to call 911, when to use urgent care, and how to create a personal emergency plan.
How much do services cost?
We offer clear monthly packages with no hidden fees. Pricing ranges from $120 to $300 per month, depending on the level of support you choose. A free consultation is available to help you decide.
Is there a long-term contract?
No. Services are offered month-to-month. You may change or cancel your package with advance notice.
Do you offer services in multiple languages?
We strive to provide culturally responsive services and can arrange interpretation or communication supports when needed. Please let us know your language or communication preferences.
